DESMOND
For people recently diagnosed with diabetes
DESMOND stands for: Diabetes Education Self Management for Ongoing and Newly Diagnosed type 2 Diabetes.
DESMOND group sessions/ MyDESMOND can help you:
- Understand more about Type 2 diabetes.
- Learn about things you can do to look after yourself and stay healthy with diabetes.
Due to COVID-19 restrictions, DESMOND group sessions are currently unavailable but there is free access for anyone with type 2 diabetes to MyDESMOND
MyDESMOND is delivered via the internet, using your computer, laptop or similar device and is a programme of education to support you to manage your diabetes on a daily basis.
You will need an access log-in code, easily obtained by contacting the team directly, using the details below, or via a referral from your GP surgery.
For healthcare staff: A referral form can be completed by the GP surgery or the patient, and then passed onto the DESMOND team
