Cornwall General Practice Fellowship – Population Health Champion

Hours: 2 Sessions Per Week - 12 Months
Salary: The salary for this post will be in line with professional scales, and reflective of this being a nonclinical portfolio role. A CPD allowance of £500 will also be provided.

Summary

Cornwall Primary Care Training Hub are pleased to support the introduction of the population health champion, a fellowship funded by Health Education England.

In General Practice we are used to working closely with individual patients to support them with their health, but we all know that there are many factors impacting on health that it is difficult for us or our patients to control, such as lifestyle and access to services. Most of us have ideas about what could be done about these bigger issues but don’t feel we have much influence. This is an opportunity for a paid portfolio role, for 1 day per week over 12 months, supported by our local Public Health Team so that you can make a real difference with these ideas.

If you are a GP, a practice nurse or another health care professional working in Cornish General Practice and want to expand your impact on patient care please contact us to discuss this exciting new opportunity.

The successful applicant will be supported by the Cornwall Public Health team, to develop or work on an existing project that aims to deliver population health improvements and address health inequalities. This might be based on a specific practice or PCN priority or a wider issue presenting the population of Cornwall. The postholder will receive educational supervision by a member of the public health team and will meet regularly with them across the 12 months. Employment and line management will be provided through Cornwall Primary Care Training Hub. The postholder will also benefit from regional education and study sessions via the HEE public health faculty, and the Primary Care Academy.

Eligibility: This role is open to all GPs, Nurses or AHPs holding professional registration and employed in general practice in Cornwall. Sessions/hours must be taken above your current clinical role unless agreed with your employing practice.
Time commitment: 2 sessions/1 day (equating to 8 hours) per week, for 12 months
