This is an exciting opportunity to provide key pharmacist expertise to GP practices within the locality. As a Clinical Pharmacist in an East Cornwall practice you will manage your own clinics to see patients for medication reviews, assist the practice team with prescribing related issues and treat patients with uncomplicated acute illness. You will help support the East PCN with core medicine optimisation activities, and ensure the safe and effective use of medicines across East Cornwall.

We will provide you with professional mentoring and bespoke coaching support so you can expand your clinical skills and confidence to best support your patients. East Cornwall Primary Care Network
The East Cornwall locality General Practices have formed a provider company Kernow Health East Ltd which will operate the new East Cornwall Primary Care Network (PCN). There are 9 GP practices in the locality which includes GP practices in Liskeard, Pensilva, Launceston, Saltash, St Germans, the Rame Peninsula, Looe, Callington and Gunnislake. East Cornwall is the largest of the 10 localities and has a registered PCN population of approximately 105,000 patients. You will be based across the locality’s GP practices providing clinical pharmacist support as part of the practice clinical teams.

You will have responsibility to support the East PCN deliver on the Network DES specification, the Improved Access to General Practice contract, and other areas of development such as research and service redesign.

We will need you to have proven skills and knowledge in:
• Creating strategic plans and delivery of implementation programmes within health care
• Establishing and evolving an effective and dynamic multidisciplinary health care teams
• Managing operational activity to deliver on key performance measures for PCN contracts and organisational priorities
• Developing and sustaining effective internal and external stakeholder relationships
• Growing and enhancing the skills and knowledge of our people to deliver high quality and safe healthcare services
