Understanding ‘Diabetes & You’ – how to support self-management for Type 2 diabetes
This module equips health care staff in primary care with essential skills to support diabetes patient self-management. The module is delivered in 2 MS Teams sessions as follows:
- 21st July, 2.00-4.00
- 28th July, 2.00-4.00
You must attend both sessions.
The module will explore participants experiences of promoting self-management and covers key messages for diabetes conversations and consultations, including dietary advice to support newly-diagnosed, and those who have had diabetes for some time.
The module draws on adult learning theory and a coaching philosophy to demonstrate how this can support diabetes self-management and introduces, ‘Diabetes & You’ the new self-management diabetes education for patients registered with Cornwall GP practices.

Prospective educators should have experience of coaching/teaching and of working with people with diabetes. Educator training is provided and consists of the Understanding ‘Diabetes and You’ module with an additional Delivering ‘Diabetes and You’ session.
