The ‘Long Term Conditions Management and Health Promotion in Primary Care’ optional module will cover:
- Understanding long-term condition diagnosis, management and review in primary care.
- Health promotion e.g. health checks
This module is delivered via a blended learning approach including e-learning and highly directed study followed by live interactive session involving presentations, case scenarios and quizzes to consolidate learning. The interactive session will be 1.5 hours delivered via MS Teams.

Booking information – when completing the booking form, you should respond ‘no’ when asked to confirm if you are signed up to the Primary Care Education Programme and then use the code ‘PASSPORT’ when asked for your Practice code.
